If you are on this page, you may be considering registering your child(ren) at Grace Lutheran Church & School. If so, please know we are excited at the possibility of having your child(ren) in one of our classrooms. Registration requires three documents to be completed and submitted to the school office. If you have questions or problems regarding these forms and the registration process, please call the school office.

1. School Enrollment Application : This is an online form. Follow the link below, complete the form and submit it to the school office. You will be given the option to download a copy of the completed form for your own records.
2. Statement of Health Form : This a PDF form for you to download and take to your child’s pediatrician to be completed and signed. This form is required for all students at Grace Lutheran School and needs to be on file as soon as possible.
3. Vision-Eye Screening Form : This is a PDF form for you to download and take to your child’s pediatrician to be completed and signed. This form is required for all students 4 years old or older and must be on file within 120 days of enrollment.
